Warm medicated oil held over the lower back within a dough ring — a targeted therapy traditionally used for chronic low-back pain and disc problems.
Kati Vasti is a localised oil therapy for the lower back. Kati is Sanskrit for the waist and lumbar region, and vasti means a container: a ring of dough on the lower back becomes a small well that holds warm medicated oil over the spine. It belongs to the Vasti family with Greeva Vasti and Jaanu Vasti, and despite the name it involves no enema.
A session runs about 30–45 minutes. Kati Vasti is carried out at Sri Vinayaga Ayurvibe, a doctor-led Ayurveda hospital at Nookampalayam, Perumbakkam, Chennai 600131 — within reach of Medavakkam, Sholinganallur, OMR, Pallikaranai, Velachery and Tambaram.
What Kati Vasti is traditionally used for
Kati Vasti is traditionally used for lower back pain, disc and sciatica complaints, inflammation and spinal stiffness.
It is traditionally used for long-standing lower back pain and the conditions behind it: lumbar spondylosis, a bulging or prolapsed disc at L4–L5 or L5–S1, sciatica that starts in the lower back, lumbar muscle spasm, and stiffness that makes getting out of bed or a chair slow.
The doctor typically plans it for people who sit for long hours, two-wheeler commuters, and adults with wear-related changes on imaging. Yogaraja Guggulu may be prescribed alongside after consultation, and Elakizhi or Naadi Swedanam added where spasm spreads into the hips.
What happens in a session
The doctor marks the area to treat from your examination and imaging, usually the lowest part of the spine just above the buttocks.
You lie face down with a thin pillow under the abdomen to flatten the lumbar curve, so the oil sits evenly instead of running to one side.
Black-gram dough is rolled into a rope and shaped into a ring about the size of a saucer, its inner edge pressed into the skin so it seals.
Medicated oil, such as Dhanwantharam Thailam, is warmed, tested on the therapist’s hand and poured in. Every few minutes the cooled oil is drawn off with cotton and replaced, keeping the warmth even for the 30–45 minutes.
The ring is lifted off and the oil massaged into the lower back and hips; you are shown how to roll onto your side and push up rather than sit straight up.
Who should not have Kati Vasti
Numbness around the saddle area, a change in bladder or bowel control, or spreading leg weakness. This can be cauda equina syndrome, a same-day emergency.
Back pain with fever, night sweats, weight loss or a history of cancer, which needs investigation first.
A recent fall, a suspected fracture or bones known to be thin, until imaging has been reviewed.
An acute flare where the back is hot or burning rather than stiff; the doctor may postpone or use a cooler approach.
Pregnancy, spinal surgery in the last few months, or broken skin over the lower back.
How many sessions?
Kati Vasti is given as a course, one session a day, because classical practice relies on oiling the same area on consecutive days. The doctor sets the length from how long the pain has lasted, the imaging and any leg involvement, and reviews it partway through by how far you can bend, how long you can sit and how the leg feels.
Where there is sciatica or long-standing pain, it may be one part of a wider plan with Pizhichil or a Panchakarma course of Matra Vasti and Kashaya Vasti.
Before you come
Bring your lumbar MRI or X-ray, with the films or CD if you have them, and a list of your painkillers or muscle relaxants.
Wear loose trousers with a soft waistband; the ring sits just above the waistline.
Tell us if lying face down brings on leg pain, so the position can be adjusted.
Afterwards
The lower back may feel warm and loose for a few hours; mention any next-morning ache at your next session.
On treatment days, avoid bending to lift from the floor and sitting for long stretches without getting up.
Wash the oil off with warm water after an hour or two, and keep the area covered from cold air.
Call +91 81229 39197 at once if new numbness, leg weakness or bladder changes appear. These are not after-effects of the oil.
Cost
Cost depends on the duration and whether the doctor plans a single session or a course. Call +91 81229 39197 and we will tell you before you book — there are no surprises at the counter.
No. The word is shared with the Panchakarma enema, but here the container is the dough ring on your skin, and no diet or bowel preparation is needed. If Matra Vasti or Kashaya Vasti is also planned, that is a separate procedure.
What if I cannot lie on my stomach?
Pillows under the hips and chest make it possible for many people. If it still hurts, the doctor may use Pichu instead, a warm oil-soaked pad applied while you lie on your side.
Should I keep wearing my lumbar belt?
If a doctor or physiotherapist advised it, keep using it as they said. The doctor may suggest wearing it less as you build up movement, but that is agreed with you.
